Functional Assessment and Fall Prevention Program

Fall Prevention Pathway

Each client with a known or suspected risk of falls will be assessed and treated according to an individualized treatment plan. The full assessment includes a complete fall history, review of medications, gait, balance, mobility, lower extremity joints, habits/routines, vision, cognition, vital signs, and home safety assessment.

Identifying Program Participants

Any person with a known or suspected fall risk is an excellent candidate for the Balance Works Program. Risk factors:

  • Muscle weakness
  • History of falls
  • Gait deficit
  • Balance deficit
  • Use of assistive device
  • Visual deficits
  • Neurological deficits
  • Arthritis
  • Impaired ADLs
  • Depression
  • Cognitive impairment
  • Age > 80 years
  • Medications
  • Cardiovascular deficits
Insurance Coverage
  • Medicare Part A covers therapy at 100% if patient is homebound.
  • Medicare Part B covers therapy at 80% with no homebound restrictions; secondary insurance usually covers the 20% once deductible and copay are met.
  • Private insurances usually accepted- reviewed on an individual basis
  • Self pay

Data determined from each assessment will be used to guide treatment using evidenced-based approaches. The end result, determined by re-evaluation, will most likely show an improvement in balance scores and a reduction in risk of falls.
